ABSTRACT

Objective:To explore the expectations and values of patients with malocclusion on participation in shared decision-making of orthognathic surgical protocols, and to provide references for further development of clinical shared decision-making models. Methods:Based on the expected value theory and descriptive qualitative research methods, using purposive sampling, 13 patients with malocclusion in the Ninth People's Hospital of Shanghai Jiao tong University School of Medicine from May to August in 2021 were selected for semi-structured interviews. The interview data were sorted, classified and refined by traditional content analysis. Results:Two themes were extracted: patients' ability beliefs about their ability to participate in shared decision making for orthognathic surgery(decision support ability, psychological coping ability and environmental adaptability), and task values for shared decision making for orthognathic surgery(interest value, acquisition value). Conclusions:Low level of patients′ ability beliefs in shared decision-making, active physician guidance facilitates patient participation, but the depth of patient participation is influenced by factors such as information support, cultural climate, and physical space for shared decision making. It is suggested that the magnetic role of physicians should be actively played, the shared decision-making team should be strengthened, at the same time, hospital manager should enhance shared decision making propaganda to increase the acceptance and participation of patients in shared decision making so as to improve the quality of shared decision-making.

ABSTRACT

Objective:To understand the problems and countermeasures of information support for dentofacial deformities patients, so as to provide reference for optimizing the whole process of information support for orthognathic-orthodontics treatment. Methods:Using a descriptive research method, 10 orthognathic surgery and orthodontics specialists from a tertiary level A hospital were selected for semi-structured interviews in August and September, 2019 using the method of target sampling. The QSR Nvivo 12.0 software was used for analysis by adopting traditional content analysis. Results:According to the analysis, medical experts believed that the problems of information support in the whole treatment process of orthognathic-orthodontics treatment include the following three aspects: information content problems (incomplete information content, lack of "time" dimension, patient's psychological expectation and treatment outcome information asymmetry, lack of information for main caregivers), difficulties in information support (clinical workload, professional limitations, many key time nodes), existing network information problems (poor quality of network information, lack of pertinence of information content and the existing network resources were not effectively utilized); and three suggestions were put forward: customizing the treatment schedule of patients, developing diversified information support methods, optimizing patient treatment process. Conclusion:Medical staff should pay attention to the construction and improvement of the whole treatment information support content of dentofacial deformities patients, pay attention to the diversity of information support methods, optimize the patient treatment process, encourage the health care to cooperate with the homogenization management of the whole treatment information support, and explore the information support system suitable for the situation of dentofacial deformities patients in China.

ABSTRACT

Objective To investigate the significance of digital tomosynthesis (DTS) for suspicions lesions in the physical examination of the chest. Methods Totally 1 000 physical examinees were divided into two groups, with 500 younger ones in one group and the remained 500 ones in the other. The examinees underwent examination with digital X-ray radiography, and then the suspicious cases went through DTS examination to analyze the detection rate of pulmonary positive results. Results There were 110 suspected cases found by digital X-ray radiography, including 8 young ones and 102 old ones. The suspected cases went through DTS examination, and totally 92 nodules were found including 4 ones in the young persons and 88 ones in old persons.CT examination found 89 carcinomatous nodules in the 92 ones. Conclusion DTS examination can detect the lesion of the physical examinee, and lays a foundation for early diagnosis and treatment.

ABSTRACT

Objective To apply high-quality nursing principles in the nursing education, allow nursing students to adapt to the modern nursing pattern more effectively in the oral and maxillofacial-head and neck oncology ward, and then to evaluate the effect of this application. Methods From June 2013 to March 2014, 60 nursing students had practiced in our department were randomly divided into the control group (30 cases, traditional teaching method) and the experimental group (30 cases, high-quality nursing teaching method). Nursing expertise, nursing procedures, teachers′ evaluation and patients′ satisfaction degree were evaluated. Results The nursing expertise, nursing procedures, teachers′ evaluation and patients′ satisfaction scores was (92.97 ± 5.65), (92.93 ± 4.38), (94.20 ± 2.95), (93.60 ± 3.97) in the experimental group respectively, while in the control group was(87.93 ± 4.62),(87.55 ± 7.54),(89.67 ± 6.21),(89.50 ± 5.80)respectively. All the four evaluation indexes of the experimental group were better than that of the control group(t=-3.78,-3.38,-3.61,-3.20), P﹤0.05. Conclusions Applying high quality nursing care principle in the nursing education allows students to adapt to the modern nursing pattern more quickly, and to establish foundations leading to their future professions.

ABSTRACT

Objective To study the mental status of dento-maxillofacial deformities patients for orthognathic-orthodontic treatment, assess the testing results, and provide reference for doctors and nurses to evaluate mental features of patients. Methods 79 patients from a three-level and first-class hospital in Shanghai were collected from February 2011 to January 2012,who were to undergo orthognathic-orthodontic treatment. All the subjects filled in MMPI questionnaire before the operation. Results Scores of F and Pa subscales in male were significantly lower than the national norm.Scores of K and Mf subscales in female were significantly higher than the national norm.Scores of L,F,Hs,D,Pd,Pa,Pt,Sc,Si subscales were significantly lower than the national norm.Scores of F,Pa,Sc,Ma subscales in male were significantly higher than female.Scores of Mf subscales in female were significantly higher than male. Conclusions The whole personality of dento-maxillofacial deformities patients are normal. However, individualized personality deviation and psychological problems exist, including personality characteristics among different genders.

ABSTRACT

Objective To research the significance of application of clinical nursing pathway in patients undertaking glossectomy and forearm flap reconstruction. Methods Sixty patients with tongue cancer who undertook glossectomy and forearm flap reconstruction were enrolled. The patients were randomly divided into the experimental group and the control group (30 cases in each group). The patients of the experimental group received nursing following clinical nursing pathway, and the patients in the control group received routine nursing service. The patients and doctors satisfaction evaluation were compared between the two groups with a self-designed nursing quality evaluation form. The qualities of care achieved in 2 groups were compared. Results It took less in-ward time and expenditure in the experimental group compared with the control group. The quality of care, the satisfactory degree of patients and doctors in the exipermental group were significantly higher than those of the control group. Conclusions The establishment and application of the clinical nursing pathway in patients undertaking glossectomy and forearm flap reconstrucition contributes to improve the satisfaction of patients and doctors and the quality of nursing service.

ABSTRACT

The present study was undertaken to observe the effect of picrotoxin(PT) injection into lateral ventricle (LV) on cardiac contractility (CC) and renal nerve discharge (RND).lt was found that PT injection into LV caused CC and RND to increase significantly, and this effect was dose-dependent.The effect of baroreflex on cardiovascular response produced by PT injection was also observed. Data suggested that PT injection into LV still caused CC and RND to increase when the buffer nerve was sectioned. The magnitude of increase was significantly larger than that of the intact buffer nerve. The results indicate that in the CNS, the block of gamma-aminobutyric acid (GABA) transmitter system not only could cause blood pressure and heart rate to increase, but also strengthen CC and RND; baroreflex could partly suppress cardiovascular response caused by PT injection.
